Pension Plan - Expression of Wishes

Is there an option on a pension plan or if not… is it possible to have a check box on a pension plan to: - 1. indicate if an expression of wishes form has been completed and 2. an area to annotate the date completed, beneficiary/s name, date of birth and percentage
